Background Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. Methods We compared preferences of (...) parents/patients at enrolment, documented by a questionnaire, with preferences developed during follow-up by an interview-study to investigate equipoise of child-participants and parents in the BeSt-for-Kids-study. This trial in new-onset Juvenile Idiopathic Arthritis-patients consists of three strategies. One strategy comprises initial treatment with a biological disease-modifying-antirheumatic-drug, currently not standard-of-care. Semi-structured interviews were conducted with 23 parents and 7 patients, median 11 months after enrolment. Results Initially most parents and children were not in equipoise. Parents/patients who refused participation, regularly declined due to specific preferences. Many participating families preferred the biological-first-strategy. They participated to have a chance for this initial treatment, and would even consider stopping trial-participation when not randomized for it. Their conviction of superiority of the biological-first strategy was based on knowledge from internet and close relations. According to four parents, the physician-investigator preferred the biological-first-strategy, but the majority stated that she had no preferred strategy. In phase 2, preferences tended to change to the treatment actually received. Conclusions Lack of equipoise during enrolment did not reduce study recruitment, mainly due to the fact that preferred treatment was only available within the study. Still, when developing a trial it is important to evaluate whether the physicians’ research question is in line with preferences of the patient-group. By exploring so-called ‘informed patient-group’-equipoise, successful recruitment may be enhanced and bias avoided. In our study, lack of equipoise during trial-participation did not reduce retention in those assigned to a less favoured option. We observed a change for preference towards treatment actually received, possibly explained by comparable outcomes in all three arms. (shrink)

Throughout the biological and biomedical sciences there is a growing need for, prescriptive ‘minimum information’ (MI) checklists specifying the key information to include when reporting experimental results are beginning to find favor with experimentalists, analysts, publishers and funders alike. Such checklists aim to ensure that methods, data, analyses and results are described to a level sufficient to support the unambiguous interpretation, sophisticated search, reanalysis and experimental corroboration and reuse of data sets, facilitating the extraction of maximum value from data sets (...) them. However, such ‘minimum information’ MI checklists are usually developed independently by groups working within representatives of particular biologically- or technologically-delineated domains. Consequently, an overview of the full range of checklists can be difficult to establish without intensive searching, and even tracking thetheir individual evolution of single checklists may be a non-trivial exercise. Checklists are also inevitably partially redundant when measured one against another, and where they overlap is far from straightforward. Furthermore, conflicts in scope and arbitrary decisions on wording and sub-structuring make integration difficult. This presents inhibit their use in combination. Overall, these issues present significant difficulties for the users of checklists, especially those in areas such as systems biology, who routinely combine information from multiple biological domains and technology platforms. To address all of the above, we present MIBBI (Minimum Information for Biological and Biomedical Investigations); a web-based communal resource for such checklists, designed to act as a ‘one-stop shop’ for those exploring the range of extant checklist projects, and to foster collaborative, integrative development and ultimately promote gradual integration of checklists. Although Archaea inhabit the human body and possess some characteristics of pathogens, there is a notable lack of pathogenic archaeal species identified to date. We hypothesize that the scarcity of disease‐causing Archaea is due, in part, to mutually‐exclusive phage and virus populations infecting Bacteria and Archaea, coupled with an association of bacterial virulence factors with phages or mobile elements. The ability of bacterial phages to infect Bacteria and then use them as a vehicle to infect eukaryotes may be difficult for (...) archaeal viruses to evolve independently. Differences in extracellular structures between Bacteria and Archaea would make adsorption of bacterial phage particles onto Archaea (i.e. horizontal transfer of virulence) exceedingly hard. If phage and virus populations are indeed exclusive to their respective host Domains, this has important implications for both the evolution of pathogens and approaches to infectious disease control. Many biosystems are complex mixtures of disparate biofluids. To study contact and transport phenomena in these mixtures, one has to apply much information on the biofluids which are components of the mixtures. A lot of the corresponding data can be extracted by means of experiments. However, it is not always easy to obtain experimental results on rather deep physical characteristics of biofluids, especially if the bioparticles are complicated systems and the fluid coexists in the mixture with a large number of (...) other fluids. In these cases, the necessary data can, in principle, be extracted from those results which are easier to obtain experimentally. The present work proposes a method to evaluate the biofluid equilibrium pressure and electrochemical potential from common experimental values of the fluid concentration and absolute temperature as well as the fluid-particle mass, volume and spin. In so doing, the nonzero values of the particle volume are accounted for. The procedure is illustrated with a numerical example on the fluid of red blood cells (or erythrocytes) in human blood. The pressure values obtained are 49.1 and 38.8 micropascals for men and women respectively whereas the electrochemical-potential values are –2.124 and –2.130 electronvolts for men and women respectively. Following his retirement fromUniversity teaching in 1972 Bocheski focusedincreasingly on metaphilosophical issues. Someof these he considered in occasional papers,autobiographical essays, as well as interviews,often giving expression to views that are asrefreshing as they are – sometimes –surprising. Bocheski in his later years becamesomething of an iconoclast, sharply criticalof, indeed hostile to, much of what isparadigmatically taken to be `philosophy'. Inthis paper, I draw out and examine some aspectsof Bocheski's virtually anti-philosophicalattitudes and try my hand at an analysis ofwhat appears to (...) have become the Bocheski'smain underlying motivation in this respect: theless than happy interplay of religious faith,Weltanschauungen, and philosophy caughtbetween two seemingly persistent butconflicting inclinations of the human spirit tosubmit to the authority of some `total' picture. The strong weak truth table (sw) reducibility was suggested by Downey, Hirschfeldt, and LaForte as a measure of relative randomness, alternative to the Solovay reducibility. It also occurs naturally in proofs in classical computability theory as well as in the recent work of Soare, Nabutovsky, and Weinberger on applications of computability to differential geometry. We study the sw-degrees of c.e. reals and construct a c.e. real which has no random c.e. real (i.e., Ω number) sw-above it.

What kind of feeling is shame? Why are we ashamed and what are the consequences that this emotion has on the subject? Is it a private or a "social" emotion? How many types of shame exist? Is vicarious shame always shameful? What are the ways in which art has represented shame? What are the relations between shame, guilt, embarrassment and censorship? Is it possible to be immune to shame? Would "Not being ashamed" count as a virtue or is it a (...) morally reprehensible attitude? Sensibilia. Colloquium on Perception and Experience investigated the different ways in which shame can be described and staged, presupposed and understood. The proposals and the points of view collected in this volume relate in an original and interdisciplinary way different areas of research all equally interested in reflecting on a complex and stratified feeling. The result was a heated debate on (ancient and contemporary) inconvenient, thorny and "shameful" issues. (shrink)

A new interpretation of quantum theory is proposed. It coincides in a number of points with the orthodox interpretation, the main difference being that the projection of the state vector can occur without the intervention of any observer.

Whooping cough is a highly transmitted disease around the world. According to the World Health Organization, 0.15 million cases had reported globally in 2018. Most of the Asian and African states are infected regions. Through the study, we investigated the whole population into the four classes susceptible, exposed, infected, and vaccinated or recovered. The transmission dynamics of whooping cough disease are studied analytically and numerically. Analytical analyses are positivity, boundedness, reproduction number, equilibria, and local and global stabilities. In numerical analysis, (...) we developed an implicit numerical integration scheme consistent with the biological problem’s properties. The analysis of the implicit method for the said model is dynamically consistent, positive, and bounded. Furthermore, an implicit numerical integration scheme is suitable for studying a particular epidemic model such as the whooping cough disease. (shrink)

The Search for the Legacy of the USPHS Syphilis Study at Tuskegee is a collection of essays from experts in a variety of fields seeking to redefine the legacy of the infamous Tuskegee Syphilis Study. The essayists place the legacy of the study within the evolution of racial and ethnic relations in the United States. Contributors include two leading historians on the study, two former United States Surgeons General, and other prominent scholars from a wide range of fields.

The Birkhoff-Maltsev problem asks for a characterization of those lattices each of which is isomorphic to the lattice L(K) of all subquasivarieties for some quasivariety K of algebraic systems. The current status of this problem, which is still open, is discussed. Various unsolved questions that are related to the Birkhoff-Maltsev problem are also considered, including ones that stem from the theory of propositional logics.

A comprehensive, critical evaluation of the ideas of the Italian philosopher Benedetto Croce, including a summary of his life.

A survey of 106 medical students assessing their interest in and attitudes to medical ethics in the curriculum is reported by the authors. Results indicate that 64 per cent of the students rated the importance of medical ethics to good medical care as high or critical and 66 per cent desired to learn more about the topic. However, in reports of patient encounters identifying ethical issues, less than six per cent of the students reported a frequency of more than one (...) such patient encounter per week. The students also demonstrated a greater awareness of more obvious ethical issues than of more subtle, less publicised issues. When asked how medical ethics should be taught, the students clearly affirmed a desire for an integrated exposure to the subject throughout the medical curriculum. We introduce the notion of an alphabetic trace of a cut-free intuitionistic prepositional proof and show that it serves to characterize the equality of arrows in cartesian closed categories. We also show that alphabetic traces improve on the notion of the generality of proofs proposed in the literature. The main theorem of the paper yields a new and considerably simpler solution of the coherence problem for cartesian closed categories than those in [11, 14].

For a first-order formula φ(x; y) we introduce and study the characteristic sequence ⟨P n : n < ω⟩ of hypergraphs defined by P n (y₁…., y n ):= $(\exists x)\bigwedge _{i\leq n}\varphi (x;y_{i})$ . We show that combinatorial and classification theoretic properties of the characteristic sequence reflect classification theoretic properties of φ and vice versa. The main results are a characterization of NIP and of simplicity in terms of persistence of configurations in the characteristic sequence. Specifically, we show that (...) some tree properties are detected by the presence of certain combinatorial configurations in the characteristic sequence while other properties such as instability and the independence property manifest themselves in the persistence of complicated configurations under localization.
